What is USMLE Step 1 and what does it cover?

Step 1 is an eight-hour, single-day, multiple-choice examination that tests your ability to apply the basic sciences to the practice of medicine. It covers anatomy, physiology, biochemistry, pharmacology, pathology, microbiology, immunology, behavioural sciences, genetics, nutrition and ageing, with a strong emphasis on mechanisms and first principles.

Is Step 1 really pass/fail now?

Yes. Since 26 January 2022, Step 1 results have been reported as pass or fail only. Programmes can no longer use a three-digit score to rank applicants, which has shifted weight onto Step 2 CK, research, letters of recommendation and US clinical experience.

If it is pass/fail, does Step 1 still matter?

For IMGs, yes. A first-time pass is still expected, and a fail is visible to programmes for the rest of your application life. Strong preparation also pays forward into Step 2 CK, where the score now carries most of the screening weight.

The Educational Commission for Foreign Medical Graduates (ECFMG) certifies International Medical Graduates as ready to enter US graduate medical education. You need it to sit USMLE Steps as an IMG, to enter the Match, and to start a US residency programme.

Confirm your medical school is listed in the World Directory of Medical Schools with an ECFMG sponsor note; apply for an ECFMG/USMLE ID and complete the application for examination; pass USMLE Step 1 and Step 2 CK; have your medical school primary-source verify your degree through EPIC; and meet the clinical and communication skills requirements.

J-1 vs H-1B: which visa is right for UK residents?

Most IMGs train in the US on a J-1 visa, sponsored by ECFMG. J-1 is more widely accepted across programmes but usually requires you to return home for two years after training unless you secure a waiver. H-1B does not carry a home-return requirement and counts toward a path to a green card, but fewer programmes sponsor it and it requires you to have passed Step 3 before applying.

Can I stay in the US after residency?

On H-1B you can transition to other employment-based visas and pursue a green card directly. On J-1 you typically need a waiver, most commonly through a Conrad 30 placement in an underserved area, before you can stay long term.

Step 1 is delivered at Prometric test centres internationally, including in London and other UK cities, so most UK students sit it without needing to travel abroad.

How do I secure US clinical electives or observerships from the UK?

Hands-on electives with patient contact are the most valuable. Apply early through university partnerships, the VSLO programme used by many US schools, and direct applications to academic centres. Observerships are non-hands-on and easier to secure, often through personal contacts, but carry less weight than true clinical rotations.

How is UK clinical experience viewed by US programmes?

UK clinical experience is respected but does not substitute for US clinical experience. Programmes want to see that you can function in a US healthcare setting, so a few well-placed US rotations with strong letters of recommendation from US-based attendings are extremely valuable.

The National Resident Matching Program (NRMP) uses an algorithm to pair applicants with residency programmes based on ranked preference lists submitted by both sides. IMGs apply through ERAS in September, interview through autumn and winter, and learn their result on Match Day in March.
